Since 2000 the A121 has seen 1,199 collisions, 18 of them fatal and 193 serious. That ranks it 401st of 6,754 numbered A, B and M roads by total recorded collisions. Its fatal rate is 15 per 1,000 collisions, and 18% caused death or serious injury. Collisions have fallen by about 46% since 2000, from 82 to 44 in 2024. The heaviest local concentrations are in Epping Forest, Broxbourne and Redbridge.
