Since 2000 the A381 has seen 1,108 collisions, 20 of them fatal and 171 serious. That ranks it 446th of 6,754 numbered A, B and M roads by total recorded collisions. Its fatal rate is 18.1 per 1,000 collisions, and 17% caused death or serious injury. Collisions have fallen by about 50% since 2000, from 60 to 30 in 2024. The heaviest local concentrations are in South Hams, Teignbridge and Torbay.
