Since 2000 the A402 has seen 2,939 collisions, 13 of them fatal and 424 serious. That ranks it 132nd of 6,754 numbered A, B and M roads by total recorded collisions. Its fatal rate is 4.4 per 1,000 collisions, and 15% caused death or serious injury. Collisions have fallen by about 54% since 2000, from 171 to 78 in 2024. The heaviest local concentrations are in Hammersmith and Fulham, Kensington and Chelsea and Westminster.
