Since 2000 the A415 has seen 1,052 collisions, 24 of them fatal and 177 serious. That ranks it 475th of 6,754 numbered A, B and M roads by total recorded collisions. Its fatal rate is 22.8 per 1,000 collisions, and 19% caused death or serious injury. Collisions have fallen by about 81% since 2000, from 72 to 14 in 2024. The heaviest local concentrations are in Vale of White Horse, West Oxfordshire and South Oxfordshire.
