Since 2000 the A523 has seen 1,086 collisions, 27 of them fatal and 175 serious. That ranks it 460th of 6,754 numbered A, B and M roads by total recorded collisions. Its fatal rate is 24.9 per 1,000 collisions, and 19% caused death or serious injury. Collisions have fallen by about 37% since 2000, from 48 to 30 in 2024. The heaviest local concentrations are in Staffordshire Moorlands, Cheshire East and Cheshire.
