Since 2000 the A645 has seen 1,119 collisions, 25 of them fatal and 221 serious. That ranks it 440th of 6,754 numbered A, B and M roads by total recorded collisions. Its fatal rate is 22.3 per 1,000 collisions, and 22% caused death or serious injury. Collisions have fallen by about 55% since 2000, from 62 to 28 in 2024. The heaviest local concentrations are in Wakefield, North Yorkshire and East Riding of Yorkshire.
