Since 2000 the A91 has seen 1,082 collisions, 47 of them fatal and 251 serious. That ranks it 461st of 6,754 numbered A, B and M roads by total recorded collisions. Its fatal rate is 43.4 per 1,000 collisions, and 28% caused death or serious injury. Collisions have fallen by about 86% since 2000, from 64 to 9 in 2024. The heaviest local concentrations are in Fife, Clackmannanshire and Stirling.
